Cancellation, Refund & Missed Appointment Policy
Please provide at least 24 hours notice to cancel or reschedule.
Late cancellations or missed appointments may be charged the full
consultation fee. Consultations are non refundable. Late arrivals may
result in a shortened appointment.
Consent to Collect Personal Health Information (PHIPA)
I consent to House of Remedys collecting, using, storing, and
maintaining my personal health information for assessment, treatment
planning, communication, billing, and legal record keeping in
accordance with Ontario privacy legislation.
Telehealth / Virtual Care Consent
I understand virtual appointments have limitations, may involve
technology risks, and may not be appropriate for every condition. I
consent to receive services through secure electronic
communication when appropriate.
Consent for Treatment of a Minor
I confirm that I am the parent or legal guardian of the minor client and
authorize House of Remedys to provide homeopathic consultations
and recommendations on the child’s behalf.

Email & Text Communication Consent
I understand that email and text messaging are not completely secure.
I consent to House of Remedys communicating with me electronically
regarding appointments, reminders, invoices, educational information,
and general follow up.
Photo & Testimonial Release
I authorize House of Remedys to use testimonials, photographs, or
reviews that I voluntarily provide for educational or marketing
purposes. My name will only be used with my permission. I may
withdraw consent in writing before future use.
Client Financial Agreement
Consultation fees are payable as communicated at booking unless
otherwise agreed. Outstanding balances may delay future
appointments.
